incorrect usage of Multiple Keigos - 二重敬語 (Nijuu Keigo)
Trying to use Keigo is generally appreciated because it shows you're putting in an effort to be as polite as possible. Using too much Keigo, however, can become too wordy and redundant. The term for using too much Keigo is called 二重敬語/多重敬語 (Double or Multiple Keigo). Although the speaker's intent may have been to be as polite as possible, 二重敬語 may be frowned upon by listeners because it is incorrect Japanese.
For example,
Often verbs that start with お or ご indicates its a 尊敬語 (Respectful form).
*note we are not focusing on nouns that start with お or ご.
Following are some examples:

お呼び（呼ぶ or call）
ご覧（見る or see）
お聞き（聞く or hear)
お書き(書く or write)
お越し(来る or come)
おっしゃる(話す or speak)
お帰り(帰る or return)

But adding the suffix [..なられる or ..られる] can also change words into becoming a respectful form.
Combining the two above is when it can cause problems.
Examples Below
For example: (Orange=First尊敬語, Yellow=Second尊敬語)

Incorrect: お呼びになられました。
Correct: お呼びになりました。

Incorrect: ご覧になられましたか？
Correct: ご覧になりましたか？

Incorrect: お聞きになられましたか？
Correct: お聞きになりましたか？

Incorrect: お書きになられています。
Correct: お書きになる。

Incorrect: 課長がお越しになられました。
Correct: 課長がお越しになりました。

Correct: おっしゃる。

Correct: お帰りになりました。

The above was 尊敬語, but this can be the same when using
謙譲語 (the modest form).
For example, I often see 拝見 (modest form of see) and
致します/させていただきます (modest form of do) used together.

Incorrect: 拝見致しましす。
Incorrect: 拝見させていただきます。
Correct: 拝見しました。
========================
Or even there are occasions where triple 敬語s can be used,
like the mixing of お (polite prefix) + 承る (modest receive) + 致しましす (modest do)
